The RN Case Manager reports to the VP of Health Services.  The RN will be responsible to follow individuals identified for episodic nursing.  The RN will attend these individual’s clinical appointments, establish plans of care, maintain required documentation and facilitate health education and training to persons served, caregivers and/or residential staff in order to ensure the health and safety of the people served.  The RN Case Manager must be able to work a flexible schedule adapted to meet the needs of the agency.  This position is a salary exempt position.


  • Attend weekly clinical meeting prepared to discuss individuals on caseload.
  • Review and collate medical information through encounter forms, PE and case studies provided by the Program Directors.
  • Attend clinical appointments with individuals being followed by the case manager.
  • Identify and provide plans of care for individuals.
  • Attend meetings with DDS that involve individuals on caseload.
  • Attend acute and post-acute facility meetings related to individuals being case managed.
  • Maintain confidentiality in keeping with HIPAA regulations and positive demeanor while successfully juggling multiple priorities simultaneously.
  • Review nursing plans of care bimonthly, after each hospitalization and as needed.
  • Assist in discharge instructions, treatments or medication changes to the Residential House Manager and all staff.
  • Identify safety risks and possible alternatives and advocate for the rights of all individuals.
  • Handle related job duties in compliance with agency’s Programmatic Policy Manual.
  • Other duties as assigned by the supervisor.
    • Ensure programs maintain the standards outlined in the Department of Developmental Services mission statement, the Survey and Certification Tool, as well as requirements of Performance Based Objectives.
    • Attend and/or participate in training of the residential staff during their staff meetings.
    • Attend nursing team meetings.
    • Implement the agency mission through service delivery, staff development, and resource acquisition.
    • Provide recommendations to your supervisor regarding ways to improve the current systems.
    • Observe all Agency policies and procedures.
    • Accept responsibility and accountability as part of the position.
    • General understanding of MAP, conduct MAP compliance reports as necessary.


  • Complete the Agency Nursing Orientation.
  • Handle all projects with a positive attitude.
  • Ability to establish rapport with the disabled population, their families, co-workers and service providers.
  • Organized, dependable and even temperament; good telephone manner.

Effective verbal and written communication skills

  • Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
  • At least one year of full-time working experience as an RN.
  • Must be 21+ years old and have a valid driver’s license. Daily use of a car on site and be available by telephone for daily contact is also required.
