  • Overview

    The Individual Supports Instructor reports to the Program Director of Shared Living, AFC & Individual Supports and provides assessment and/or teaching of community living skills to developmentally disabled persons that facilitate the individual's ability to remain in current home environment.  The Supports Instructor delivers direct training to individuals in compliance with Agency policy and procedures. This position will be compensated on an hourly basis.



    • Provide home visit to assess new referrals and implement Individual Skills Support strategies weekly.
    • Maintain daily activity statistics and documentation as required by Agency and contract policy and procedure.
    • Participate in multi-disciplinary meetings for planning and review of Individual Support Plans.
    • Participate in all required staff and Agency meetings.
    • Provide direct individual training in community, of community living skills and defined by program contract.
    • Attend workshops and other continued education programs.
    • Report to the Program Director of Shared Living, AFC & Individual Supports for accountability of scheduled hours.
    • Compose reports as required by Agency and contract policy and procedures
    • Maintain confidentiality and positive demeanor while successfully juggling multiple priorities simultaneously.



    • Presence in administrative office; least once weekly
    • Provide recommendations to your supervisor regarding ways to improve the current systems
    • Observe all Agency policies and procedures
    • Accept responsibility and accountability as part of the position
    • Maintain confidentiality and positive demeanor while successfully managing multiple priorities simultaneously.
    • Maintain current CPR and First Aid Certifications.
    • Other duties as assigned.



    • Effective oral and written communication skills.
    • Ability to establish professional rapport with developmentally delayed population, their families and maintain service providers.
    • Possess impeccable time management and organizations skills.
    • Ability to make and keep appointments in the community.
    • Ability to analyze tasks for step by step instruction.
    • Ability to communicate effectively with staff, individuals and other service providers.
    • Ability to report performance to Family Support Program Manager.



    • at least 2 year's experience in Human Services
    • Must be 21+ years old, have a High School Diploma or GED, a valid driver’s license, daily use of a car on site and be available by telephone for daily contact.

    Employee must possess the knowledge; skills and abilities listed above, and demonstrate that they can perform the essential functions of the job. Employee must be able to speak, read and write in fluent English.
